About Company:
St. Anthony’s Hospice was founded in 1982 in Henderson County, KY by a local board of directors, spurred by the vision of John Conn and Marianne Walker. Volunteers on behalf of St. Anthony’s Hospice began providing care to individuals with life-limiting illnesses and their families in January of that year. Within one year, St. Anthony’s was employing nurses to provide hospice care. The organization grew both clinically and administratively during the first decade and expanded to serve families living in Union County in 1989 and Webster County in 1992.
The organization began aggressively expanding the reach of its mission in 2006. Since then the organization increased its service to individuals living in their own homes and in local long-term care facilities and built the St. Anthony’s Hospice Lucy Smith King Care Center in 2009.
In 2016, St. Anthony’s noticed another target population in need of extra care at home. This population is still seeking aggressive or curative treatment for his/her serious illness but is in and out of the hospital a lot. St. Anthony’s developed a Transitional Care program focused on quality of life, educating our patients about their disease process, and developing goals of care. On February 6, 2017, we admitted our first Transitional Care patient, which is now our Palliative Care program!
No matter how St. Anthony’s Hospice grows and changes with healthcare trends, we always keep to the original vision of our founders- exceptional care for those at the end of life. We believe the end of life is as precious and fragile as the beginning.
Hospice Care Aide – Care Center - Day Nights - PRN Classification: Non-Exempt (PRN)
Reports To: Clinical Director
Position Summary:
The Hospice Care Aide provides compassionate personal care to patients receiving hospice services in their private homes or residential facilities (such as nursing homes or assisted living). Care is provided according to the patient’s individualized Plan of Care and under the direction of the assigned nurse and/or physician.
Provides personal care services such as bathing, grooming, dressing, toileting, feeding, oral hygiene, and assistance with mobility in the patient’s home or facility setting.
Assists with monitoring and reporting changes in the patient’s condition, including vital signs, intake/output, and other observations, under the direction of the Registered Nurse.
Supports comfort and dignity by assisting with positioning, range-of-motion exercises, ambulation, and safety with assistive devices.
Completes light housekeeping tasks that contribute to a safe, clean care environment (e.g., making beds, tidying the patient area, removing used linens, etc.).
Documents care provided and observations accurately and in a timely manner, according to agency protocols and the Plan of Care.
Maintains respectful, compassionate communication with patients, caregivers, and interdisciplinary team members.
Travels to patient homes and care facilities as assigned.
Must be a Certified Nurse Assistant (CNA) or have previous relevant patient care experience.
Must demonstrate a commitment to the philosophy of hospice care.
Must be able to work independently in patient homes and facilities.
Must meet all legal and regulatory requirements including state/federal guidelines, Medicare Conditions of Participation, and infection control protocols.
Must be comfortable using a tablet or mobile device for clinical documentation.
Must be able to drive to multiple patient locations across Henderson, Union, and Webster counties.
Must be able to lift and move up to 50 pounds multiple times per day for patient mobility and care tasks.
Regularly exposed to patient care environments which may include blood, body fluids, and household cleaning chemicals—PPE will be provided and must be used appropriately.
Must be able to use basic medical equipment and technology for documentation and communication.
